Required of ALL New Students

 1.  Birth Certificate
      Required of all incoming students - due at time of application submission.

2.  Baptismal Certificate 
     Only if applicable - due at time of application submission

3.  Immunization Records
     Immunization record or Certificate of Religious Exemption form required for all incoming students.

4.  Student Health Form (Click here for a copy)
     Must be dated within one year of acceptance - due prior to the first day of school.      

5.  Emergency Contact Form for Prekindergarten / Preschool (Click here for a copy) 

Required for Kindergarten

 For Kindergarten - Health and Immunization requirements for school entry
 (Due prior to the first day of school)
 

Required for Students in First - Eighth Grade

1.  Two years (or one if applying for First Grade) of report cards

2.  Copies of standardized testing

3.  IEP or SAP (if applicable)

4.  For Kindergarten - Rising Kindergarten Recommendation Form
    This should be filled out by the child's teacher and returned directly to St. William of York.

5.  For First Grade - Rising First Grade Recommendation Form 
     This should be filled out by the child's teacher and returned directly to St. William of York.

6.    For Second - Eighth Grade - Recommendation Form 
         This should be filled out by the child's teacher.
